LOOKING FOR A REFUND?
To request a refund, please provide us with the below information. Refunds can only be issued to the original card purchaser. If you received the card as a gift, please contact the original purchaser of the card so they can fill out the below form and organize the refund. Please note that all fields with * are mandatory.
We will be refunding any outstanding balance of the card as of 7/9/14 by check once your original purchase has been verified. All card purchasers will be reimbursed in a timely manner to the best of our ability. If you have any questions, you may contact customer service at email@example.com.
